The illness experience of adolescent patients with systemic lupus erythematosus:a qualitative study

Objective To understand the illness experience of adolescent patients with systemic lupus erythematosus,so as to provide references for clinical medical staff to formulate targeted intervention strategies. Methods Semi-structured interviews were carried out from January to April 2022 by phenomenology. 11 adolescent patients with systemic lupus erythematosus from the pediatric clinic or ward of a tertiary A hospital in Beijing were recruited,and Colaizzi’s 7-step analysis method was applied to analyze and refine themes of interview data. Results 4 themes were extracted. Theme 1:bad physiological experience(pain and fatigue); theme 2:negative psychological experience(suffer harm from others,prejudice against oneself,fear of disease,and worries about the future); theme 3:disease coping style(conflict and resistance,compromise and concession,acceptance and adjustment); theme 4:po-sitive psychological experience(feel the support of others and gain self-growth). Conclusion Pediatric medical staff should pay attention to the physical and mental feelings of adolescent patients with systemic lupus erythematosus in the course of illness,and give scientific and effective interventions to promote their physical and mental comfort.

Early postoperative sequential enteral nutrition management in 2 children with idiopathic biliary perforation

To summarize the management experience of early postoperative sequential enteral nutrition in 2 children with idiopathic biliary perforation. Key points of nursing included establishing a special nutrition management team,strengthening nutrition screening and evaluation,formulating individualized parenteral nutrition plan,starting the early sequential enteral nutrition support process during the perioperative period,strengthening feeding tolerance assessment. Digestive fluid reinfusion was implemented,and percutaneous endoscopic gastrostomy management was strengthened;regular follow-up and continuous home enteral nutrition nursing were carried out. After careful treatment and nursing care,the nutritional status of the 2 children was significantly improved and they recovered smoothly. The growth and development were normal at the 6-month follow-up after discharge.

Summary of the best evidence for self-management in patients with chronic heart failure

Objective To retrieve,evaluate and summarize the evidence of self-management of patients with chronic heart failure,and to provide evidence-based bases for medical staff to conduct self-management health education and standardize patients’ self-management behavior. Methods We searched UpToDate,BMJ Best Practice,Joanna Briggs Institute(JBI),National Institute for Health and Clinical Excellence(NICE),Guidelines International Network(GIN),National Guideline Clearinghouse(NGC),Scottish Intercollegiate Guidelines Network(SIGN),Cochrane Library,PubMed,SinoMed,CNKI,Wanfang Database to collect relevant clinical decisions,evidence summaries,syste-matic reviews,guidelines,expert consensuses and original articles published from the databases establishment to July 14,2022. There were 3 researchers who evaluated the quality of the literature that met the quality standards,and 2 researchers extracted and integrated evidence. Results A total of 18 pieces of literature were included,including 1 clinical decision,10 guidelines,2 systematic reviews,3 randomized controlled trials and 2 expert consensuses. Finally,39 pieces of evidence were summarized in 9 aspects,including multidisciplinary team cooperation,learning disease-related knowledge,medication management,symptom monitoring,exercise training,dietary and nutrition,lifestyle,psychological adjustment,medical treatment and follow-up. Conclusion This study summarizes the best evidence of self-management of patients with chronic heart failure,which can provide references for clinical medical staff to guide self-management of patients with chronic heart failure in a more comprehensive and scientific way.

This article summarizes the experience in the nursing care of a child with fibrinogen deficiency combined with ligneous conjunctivitis. Essentials for nursing care:implementing individual medication management,designing a disease evaluation form for continuous assessment,strengthening nutritional guidance and health care intervention to promote the growth and development of children,carrying out psychological nursing to reduce the negative emotions of children and parents,implementing individualized discharge guidance and continuous nursing to promote the disease management ability of children’s family members. After 9 days of careful treatment and nursing care,the child was discharged and in stable condition within the next 31 months.

Objective To translate the short version of Glaucoma Medication Self-Efficacy Scale(GMSES) into Chinese,and to evaluate its psychometric properties in Chinese glaucoma patients. Methods After obtaining the consent of the author of the original scale,researchers conducted the translation and cross-cultural adaptation of the short version of GMSES in accordance with Brislin translation model,and the expert consultation and pilot test were also completed before full psychometric testing. A total of 244 glaucoma patients from 3 hospitals in Zhejiang province and Henan Province who received eye drops were recruited by convenience sampling. The reliability and validity of the Chinese version of the short GMSES(C-GMSES) were evaluated. Results For the overall C-GMSES,the Cronbach’s α coefficient was 0.837 and the test-retest reliability was 0.938. The Cronbach’s α coefficients of 3 dimensions were 0.741,0.933 and 0.817,respectively. The scale-level average content validity index(S-CVI/Ave) was 0.990 and the item-level content validity index(I-CVI) ranged from 0.833 to 1.000. A total of 3 common factors were extracted from exploratory factor analysis;the cumulative variance contribution rate reached 63.642%. Conclusion The C-GMSES has been proved to be reliable and valid,and it can be used to evaluate the medication self-efficacy of patients with glaucoma in China.

Application of relayed parenteral and enteral nutrition support assisted by digestive fluid reinfusion in the nursing care of 3 children with high flow fistula

To summarize the clinical nursing experience of 3 children with high flow fistula of digestive tract who underwent gastric fluid,bile fluid and intestinal fluid reinfusion combined with relay-type nutritional support. The key points of nursing include:nutritional risk screening and assessment,implementation of relay nutritional support treatment,enteral nutrition combined with digestive fluid reinfusion,fistula management and humanistic care for children and their parents. After careful treatment and nursing care,the weight of the 3 children increased steadily and their nutritional status improved gradually. All of them recovered and were discharged from hospital smoothly.

With the previous transfer experience of 3 neonates delivered by mothers with suspected coronavirus disease 2019(COVID-19) and the review of the relevant literature,the process of newborn referral is developed,including preparation and implementation of referral. Preparations before referral covers management of transfer personnel,establishment of transfer agencies,the set-up of green channels,etc. The specific implementation of referrals covers 7 aspects,including referral evaluation,grading of referral patients,principles of admission,referral personnel and material preparation,admission,and transfer out,etc. The purpose of this article is to ensure that newborns who are suspected/confirmed with COVID-19 are delivered safely,effectively,and quickly,to cut off the transmission of COVID-19 in newborn referrals,and to protect the life and health of newborns and medical staff.
